The Fund is obliged to send an information about funds available in the account to each Fund Member once a year. Customers receive correspondence by mail to the correspondence address indicated by them. It is also possible to activate an e-mail service to receive information about financial resources accumulated in the Fund in an electronic form. You can activate service by providing your e-mail address vie the following hotline: 801 10 10 (for persons calling from Poland) or 22 539 23 23 (for persons calling from abroad and for mobile phones), or by sending a completed and signed "Consent for the receipt of electronic correspondence" available at: http://www.pkopte.pl/eko-wydruk/ to the following address: PKO BP Finat Sp. z o.o. ul. In order to divide funds accumulated during the marriage, former spouses can enter into a civil-law agreement regulating the partition of funds for the period of community property. Such an agreement signed by both spouses has to be sent to the Open Pension Fund.

Open Pension Fund account should be credited with contributions for periods in which revenue which constitutes the basis for calculating contributions to the pension insurance was obtained. Contributions will not be credited e.g. for periods of unpaid leave, for periods of agricultural activity or after the insured person have received in one year revenue constituting the basis for calculating contributions in the amount which exceeds thirtyfold the average wage.

If correct data of an Open Pension Fund Member are in possession of the Open Pension Fund and of the contribution payer, the Open Pension Fund Member can submit a complaint regarding the lack of contributions. Such a complaint can be filed personally or by mail to a branch of the Social Insurance Institution (ZUS), with the use of a special form available at: http://www.pkopte.pl/ofe/brak-skladek-na-rachunku/

The draw is conducted by the Social Insurance Institution (ZUS) and refers to persons who start their first job for which pension insurance contributions are paid. Persons who began such a job should choose an Open Pension Fund within 7 days from the moment of the entry into employment. If they don't do that until 10th January or 10^th July accordingly, they are subject to a draw to Open Pension Funds conducted by ZUS.

Funds which participate in the draw are those which obtained rates of return higher than weighted average rate of return of all funds in the last two reference periods, with the provision that net assets of a given fund should not exceed 10% of all Open Pension Funds market assets.

In order to report the death of a Fund Member, one should send a written information about the death of the Member and attach an abridged copy of death certificate.

After having received the documents, the Fund will verify the situation at the Member's account and send to persons concerned appropriate information related to the possibility of payment of the deceased person's funds.

PKO BP Bankowy Open Pension Fund accepts documents certified by:

• Notary
• Institution authorised to perform such activities (e.g. local branch of the Social Insurance Institution, commune or poviat authorities, diplomatic post),
• Employee of PKO BP Bankowy PTE,
• Employee of PKO BP Finat sp. z o.o.
• Such a confirmation that a given document is a true copy has to include a seal with the name of the institution, legible name and surname of the certifying person as well as the date on which the document was certified.

An intention to change an Open Pension Fund can be declared at anytime by signing an agreement with a new pension fund. However, the transfer of means to new pension funds takes place only 4 times a year during transfer sessions which are specified in Article 122 of the Act of 28th August 1997 on the organisation and operation of pension funds and which take place in February, May, August and November.

Transfer of means between funds is free of charge.

Yes, a minor can be designated by the Fund Member as the authorised person in the event of death.

You can check funds available in your account and review the history of contributions transferred from the Social Insurance Institution to your account in the Fund at any time. In order to do that, you have to log to the Virtual Customer Office at www.pkopte.pl. For the purposes of logging to the service, the number of the account in the Fund and a TELECODE is required.

Account and telecode numbers are included in one of the first letters sent after you have opened an account in our Open Pension Fund.

Persons who are to inherit funds in an Open pension Fund should be designated with the use of a special Designation or Change of Authorised Persons Form available at www.pkopte.pl. The form has to be collected and signed, and afterwards sent by mail to PKO BP Finat to the address: ul. Grójecka 5, 02-019 Warszawa. Forms sent by fax or e-mail will not be taken into consideration, as such a change must be confirmed with an original signature of the Fund Member.

Changes can be made with the use of a Personal Data and Property Regime Change Form available at www.pkopte.pl.

Completed and signed form should be sent by mail to the following address of PKO BP Finat: ul. Grójecka 5, 02-019 Warszawa. Forms sent by fax or e-mail will not be taken into consideration, as such a change must be confirmed with an original signature of the Fund Member.
